Sickly Clown Loach

I have clown loach which is in a very condition

It has swamm into the filter three times and it forms is very mishaped and it looks very thin.

It show the classic signs of a loach resting, but it looks very disstressed

Any hints?

What colour does it have? Is it grey? I had one just like this and put
it down to a parasite - unfortunately it died :-( Does it have any white
sugar/salt like spots? If so you can probably treat it with an Ich/White
spot treatment. IME the best place to look is:
