I have a male guppy. He has a black spot on his side that seems to be on
both sides.

But it disappears for periods of time. Leaving a normal, un marked patch of
"skin" there.
Is this something to worry about?

No,
Quite a common marking, often with a small red stripe nearby. may of
my males have this, most lose it as they get older, seems to be on the
red fantail strain, non of my blue fantails or snakeskins have this
marking.

I've seen that marking on quite a few male gupps. Remember that some
gupps can alter their skin color to some degree, that may be the reason for
the patch appearing and dissappearing.
