Well-established 105 gallon tank, African cichlids (mbuna), lots of
filtration (2 Fluval 405s), and lots of holey rock - the rock is sharp and
has lots of jagged edges - one of the plecos, otherwise healthy, has what
looks like a scrape or lesion about 2\3 of the way down the side of his
body - looks like a sore that's not healing. No unusual behavior, seems
regularly active, etc. - what might this be? Any suggested treatment
regimen?

Could you post a picture?
It could be just what you said; a lesion.
If it is not too hard to catch you might try a dip in methylene blue at
double strength of the directions for 10-20 minutes. Melafix is
relatively safe to treat the entire aquarium with if a hospital
aquarium is not avaiable. If the lesion seems to be growing with any
margin around it, i would treat with Pimafix or Triple Sulfa.
There are a lot more possiblities of treatment, but I do not want to go
there yet until a better diagnosis can be made.
