Depends on what you want to do and how much time you want to spend ;o)
at a minimum:
Results of water tests whenever you do them
additions / deaths / disappearances of fish and plants
times and amounts of water changes
filter cleaning / media replacement
unusual behavior by fish
if you want to breed fish:
all of the above plus bi-weekly water tests, as many as you can (pH,
GH,KH,Ammonia, Nitrite, Nitrate)
daily or more frequent temp readings
photoperiod (how long are the lights on)
feeding times, amounts, and what was fed
appearance and behavior of fish
count of eggs / babies ;o)
date of spawn
survival and growth rate of fry
