Nitrates are very low but phosphates could be lower. I will do a water
change this weekend and get a phosphate remover in there.

I will also get some tweezers and see how much of the stuff I can pull off?

eyes are clear - skin clean today and every day he eats like a machine
(the fish is a vacuum cleaner!!!).

I think it just comes and goes if he gets a bit stressed....

My regal tang (hepatus tang) had ich last time maybe 2 years ago.
In between it was stressed many times. Last December I had a tank
crashed due to some microorganisms (dinoflagellates) and had to
redo it completely. Used this occasion to buy bigger glass...
This tang was 4 months in a plastic 20g rubbermaid container
during the time I had to wait for ordering and seting up new tank.
No ich...

I would say your fish is unhappy - maybe malnutrition ?
How often do you feed it seaweed ? It should be main
item in his diet. It provides vast number of minerals
vitamins and has better nutritional value than plankton alone.

Also, I would check water parameters.
How high are nitrates and phosphates currently in your tank ?
