How about a 50% water change with treated, temperature matched water
following a really good gravel cleaning.

What are the tank parameters?
How big
What temp
Ammonia, nitrite, nitrate
pH
filters, air stones, etc...

Does he also look skinny? I'm concerned he may not have got enough food in
your tank, this happened to my Pleco. He kept my aquarium so free of
algae, I then supplemented with algae wafers probably too late and I think
he died of starvation. :(

The Pleco does eat the available algae in the tank but I also feed Spirolina
disks and lettuce so I don't think he's starving. I think he's just a sick
fish but I can't for the life of me think what he may be sick from. As I
said before, this fish was bought for me by my fiancé and he didn't come
cheap either. I don't see to many Royals for sale in my neck of the woods so
I imagine he will be difficult to replace if I lose him.
Thanks for your advice and any more you may have would be appreciated.
