Friend's lionfish is sick - please help!

Have you tested for nitrate?

Additional info: she says it is a Volitan lionfish. She also recently
lost a 3 yr old fuzzy dwarf.

She claims nitrate is zero. She has done several water changes. She
suspects that the fish has internal parasites.
