Default Disease diagnostic chart

Courtesy of a hard drive melt down, I lost a wonderful reference for
diagnosing diseases in goldfish and how to treat them. It came from someone
who posts here but I don't recall who. I've been gone for some time while I
reconstruct my computer.

Can anyone help?
